Adapt Integrated Health Care

Adapt Integrated Health Care, founded in 1971 and headquartered in Roseburg, Oregon, has evolved into a leading nonprofit provider delivering fully integrated healthcare across Southwest Oregon. Originally focused on addiction prevention, it now encompasses a broad continuum of services: substance use treatment, mental health care, primary care, and robust community outreach.

At the core of Adapt’s model is its continuum of addiction care, which includes medically monitored detox, inpatient and residential treatment, intensive outpatient programs (IOP), opioid treatment programs (methadone, buprenorphine, naltrexone), transitional housing, day programs, and a sobering center coordinated with law enforcement. Clients benefit from evidence-based therapies cognitive behavioral therapy (CBT), motivational interviewing, contingency management and are supported with family and aftercare services .

In mental health, Adapt serves as the Local Mental Health Authority in several counties, offering 24-hour crisis care, forensic services, outpatient psychiatry, school-based programs, and specialized teams like ACT, EASA, and IIBHT.

Their primary care services, recognized as an FQHC since 2012, delivered roughly 53,000 visits in 2023 providing preventive care, chronic disease management, and telehealth to about 7,500 adults.

Adapt’s holistic, client-focused approach includes mobile clinics for rural access and a sliding-scale fee structure ensuring no one is denied care due to inability to pay . Concurrent upgrades to their Epic-based EHR system underscore their commitment to integrated, coordinated care.

Today, Adapt continues expanding its footprint with initiatives like the upcoming Recovery Campus a purpose-built facility on 14 acres and innovative services like its sobering center.
